What Can You Offer Me? - Family Service Agency provides outpatient behavioral/mental health and substance abuse treatment services to individuals and families looking for assistance in resolving their mental health issues.
What Can I Expect? - Services are provided in a professional, culturally competent and respectful manner. All our counselors are licensed by the Arizona Board of Behavioral Health Examiners and all our clinics are licensed by the Arizona Department of Health Services.

Services are available in both English and Spanish.
All services provided are treated with the utmost confidentiality – fully adhering to all H.I.P.P.A. regulations.
What Can I Get Help For? - We can assist with a variety of disorders, i.e.:
- Anxiety
- Depression,
- Grief,
- Family/spousal/partner conflict,
- Mood disorders,
- Transgender issues
- Work-life issues,
- Alcohol/drug use/abuse, etc.
These services are provided in an individual or group setting, with or without other family members or significant others, and if necessary we can provide psychiatric assessment and medication monitoring.
Services offered are:
- Assessment
- Treatment Planning
- DUI/DWI programs
- MVD evaluations
The Substance Abuse services are designed to provide counseling to individuals in a group setting. Family Service Agency encourages the client’s use of community self-help and 12-step programs.
Substance Abuse Services include:
- Relapse prevention and recovery;
- Analysis of triggers and risk factors;
- Stress management coping skills; and
- Development of healthy lifestyle choices.
What Do I Have to Do? - Whether you are seeking mental health or substance abuse counseling you will be actively engaged in determining and understanding your diagnosis and development of a treatment plan which will identify your options and course of action that you can engage in to resolve your issue(s) and return your life to a normal, peaceful, healthy functioning level.
What’s it Going to Cost Me? - We offer a competitive and affordable sliding fee scale for those without insurance with the exception of MVD assessments which are currently $125.00 per assessment.
Where Do I Have to Go for Services? - Clinics are conveniently located near your work or home in:
- Central Phoenix (Central & Virginia),
- Phoenix MetroCenter (31st Avenue & Peoria), and
- Mesa (Gilbert & Southern).
When are You Open? - Daily office hours are Monday through Friday, 8:00 a.m. to 5:00 p.m. with evening and weekend hours by appointment.
